ABSTRACT

Objective:To investigate the current status of knowledge expectation satisfaction of orthopedic patients during hospitalization, and to analyze the main influencing factors.Methods:From March 2021 to July 2021, a total of 320 orthopedic inpatients from Affiliated Hospital of Qingdao University were selected by convenient sampling method, who were investigated by the general information, Patient Participation Ability Questionnaire, Excepted Knowledge of Hospital Patients and a self-designed questionnaire of actual accepted knowledge on the basis of this scale.Results:The highest level of knowledge expectation satisfaction in the functional dimension (-0.40 ± 0.32), and the lowest level in the economic dimension (-1.27 ± 0.38) ( t=22.45, 59.64, both P<0.01). Knowledge expectation difference was positively correlated with information acquisition ability ( r=0.802, P<0.01) and emotion management ability( r=0.671, P<0.01). Multiple linear regression analysis showed that education level, hospitalization experience, information acquisition ability and ability to manage emotions were the main influencing factors for this difference ( t values were -6.40-13.17, all P<0.05). Conclusions:The level of knowledge expectation satisfaction of orthopedic patients′ needs to be further improved. According to the level of education, hospitalization experience, information acquisition ability and emotion management ability of patients, nurses should formulate measures to improve the level of knowledge expectation satisfaction.

ABSTRACT

Objective:To explore the effect of the application of nursing decision support information system in the continuing nursing of stroke patients in convalescent period, and to provide guidance for the information-based whole process nursing of stroke patients in convalescent period.Methods:A total of 107 stroke patients in convalescent period admitted to 4 Grade Ⅲ Level A hospitals of Beijing city from March to November 2019 were selected. The patients were divided into control group (53 cases) and test group (54 cases) by coin tossing method. The control group followed uniformly formulated discharge health education manual for post-discharge management and follow-up, while the test group received health guidance and follow-up through the nursing decision support information system. Barthel Index and MOS SF-36 were used to evaluate the activities of daily living and quality of life of the two groups of patients before intervention and 3 and 6 months after the intervention, and the results were compared.Results:There was no significant difference in Barthel Index before the intervention between the two groups( P>0.05). After 3 months of intervention, the Barthel Index ≤ 49, 50-70 and ≥ 71 in the test group were 7, 17 and 27 cases respectively, and 16, 21 and 13 cases in the control group respectively, and the difference between the two groups was statistically significant ( Z=-2.95, P<0.01). After 6 months of intervention, the Barthel Index ≤ 49, 50-70 and ≥ 71 in the test group were 7, 12 and 32 cases respectively, and 10, 15 and 25 cases in the control group respectively,and the difference between the two groups was statistically significant ( Z=-2.21, P<0.05). There was no significant difference in MOS SF-36 before the intervention between the two groups( P>0.05). After 3 and 6 months of intervention, the total score of MOS SF-36 in the test group was (50.51 ± 14.57), (57.06 ± 14.85) respectively, and that in the control group was (42.02 ± 15.48), (45.58 ± 14.97) respectively, and the differences between the two groups were statistically significant ( t=2.84, 3.23, both P<0.05). Conclusions:The application of nursing decision support information system can effectively improve the daily life ability of patients, enhance the quality of life of patients.

ABSTRACT

Objective To review the clinical characteristics and risk factors of Pseudomonas aeruginosa bloodstream infections. Methods The clinical data of P. aeruginosa bloodstream infections in the First Affiliated Hospital of Soochow University from January 2007 to December 2016 were analyzed retrospectively. Results Of the 251 patients identified, APACHE Ⅱ score on admission was 11.5±5.2. Majority (98.4%, 247/251) of the patients had fever. Leukopenia was found in 125 patients, leukocytosis in 87 patients, neutropenia in 122 patients, agranulocytosis in 113 patients, anemia in 193 patients, and hypoalbuminemia in 120 patients. Overall, 219 patients had at least one underlying disease, primarily hematological malignancy, or malignant solid tumor. Most (229) patients received invasive procedures such as deep venous catheter, urinary catheter, mechanical ventilation before blood sampling. P. aeruginosa was isolated from 108 of the 173 deep venous catheters. In addition, 130 patients received radiation or chemotherapy. Immunosuppressive agents were used in 124 patients. Among the 251 strains of P. aeruginosa, 87.3% were susceptible to amikacin, followed by ciprofloxacin (85.7%) and cefepime (81.6%). Multidrug-resistant P. aeruginosa was isolated from 36 (14.3%) patients, and extensively drug resistant strain was isolated from 7 patients. All the 251 patients were treated withantimicrobial agents, mainly β-lactam/β-lactamase inhibitor combinations, carbapenems or fluoroquinolones. Overall, 20 (8.0%) of the 251 patients died, 37 (14.7%) refused further therapy due to worsening condition, and 194 (77.3%) improved. Binary logistic regression analysis showed that high APACHE Ⅱ score on admission, anemia and hypoalbuminemia were risk factors for poor outcome of bloodstream infectionscaused by P. aeruginosa. Conclusions P. aeruginosa bloodstream infection occurs more readily in immunocompromised patients. High APACHE Ⅱ score on admission, anemia and hypoalbuminemia are associated with poor prognosis. Appropriate empiric antimicrobial treatment as early as possible can improve the prognosis of P. aeruginosa bloodstream infection.

ABSTRACT

Objective To assess the value of LEEP for diagnosis and treatment in patients with cervical lesion.Methods 128 patients with cervical lesion were treated by LEEP.The transformation zohe of the cervix was completely excised in every case.All specimens after LEEP were sent for pathological diagnosis.The duration of the management,the amount of bleeding,the complains of the patients as well as the close follow up after treatment were all noted and recorded in detail.Results The procedure time of LEEP was short and about 6.2 minutes without anesthesia.The bleeding of LEEP was 8.4ml,the complications were few and the cure rate was high.Conclusion LEEP is safe and effective method in treatment of cervical diseases.Standardization of operation procedure and the follow-up can get satisfactory effects.
